Certified Marriage License or Affidavit of Marriage License Due to intelligence Reform & Anti Terrorism Act of 2005, The Social Security, D.M.V. and Immigration office also require a License or an Affidavit of the License in order to change your name (This is the document obtained prior to the marriage ceremony and is not proof of marriage.
